APNP Hospitalist Program Ministry Eagle River Memorial Hospital
Critical Access Hospitals (CAH) are currently heavily reliant upon a small cadre of primary care physicians to provide inpatient services. Loss of even one physician from the call rotation can often threaten hospital inpatient coverage. This leads to chronic uncertainty and risk for CAH inpatient staffing. Ministry Eagle River Memorial Hospital (MERMH) became the pilot site for a new model of inpatient care whereby Advanced Practice Nurse Practitioners (APNP) serve as the attending providers, with physician consultation via telemedicine and phone. MERMH received a three year variance waiver to the provisions of Section DHS 124.05(2)(g)1, which requires that "a person may be admitted to a hospital only on the recommendation of a physician, dentist or podiatrist, with a physician designated to be responsible for the medical aspects of care". This variance allows APNP's at MERMH to admit patients for inpatient care and be responsible for the medical care during the inpatient stay. The presentation describes the process of program development and implementation, including the regulatory components, and also provides an overview of program outcomes to date.
