Rural Wisconsin Chronic Disease Toolkit
The
Rural Wisconsin Chronic Disease Toolkit was initially developed as part of the Rural Wisconsin Health Cooperative (RWHC) Primary Care Improvement Project, a HRSA Network Development Grant-funded collaboration between RWHC, participating RWHC member hospitals, the Wisconsin Collaborative for Healthcare Quality (WCHQ), and other partner organizations.
The Toolkit is a compendium of rurally relevant best practices that have been shown to be effective in improving chronic disease outcomes. Contents include resources for Diabetes Care, Blood Pressure Control, Effective Teams and Communication, and Care Coordination, among others. The toolkit was last updated in April 2022 and will continue to be updated over time with additional resources as they become available.

The Rural Wisconsin Caregiver Toolkit
The Rural Wisconsin Caregiver Toolkit is a compendium of publicly available resources intended to help caregivers find the information they need to effectively perform their caregiving roles. It consists of: (1) resources to help caregivers perform a variety of medical and nursing tasks ; (2) resources relating to advance directives and power of attorney; (3) links to a broad range of Wisconsin and national caregiver resources; and (4) resources intended to help caregivers address their own health and wellbeing. The Toolkit is accompanied by a separate ADRC Resource Guide, which identifies all of the ADRC's operating in Wisconsin and provides links to each ADRC’s local resource directory.
