Hospital to Hospital (H2H) Programs

Subsequent to futurist Leland Kaiser’s keynote at Wisconsin’s 2005 Annual Rural Health Conference, the RWHC Board grappled with one of Dr Kaiser’s key recommendations—that strong rural hospital leaders routinely seek out and visit other hospitals to gain additional insights to enhance their organization’s performance. The Board’s discussion over several month’s led to the RWHC Hospital to Hospital Program (H2H). Please nomination your own facility or another to RWHC’s Executive Director. We have now been doing H2H for a long enough time that return visits or repeat topics may be needed and are totally appropriate. RWHC will not share with anyone whether the hospital was nominated or self-nominated. In either case, we need the name of the hospital, the name of the hospital initiative and a few sentences about why the hospital and program is likely to be an interesting site visit. If you nominate someone else, you don't need to check if they are interested, we will do that.

Columbus Community Hospital – Focus on Performance Improvement: A Systematic Approach to Addressing Employee Opinion via Departmental Action Plans (2 Parts)

In 2004, Columbus Community Hospital (CCH) began conducting an employee opinion survey in an effort to identify and improve upon a number of organizational issues. HR Solutions, a reputable, nationally recognized survey firm conducting approximately 500 hospital surveys nation-wide was hired to complete this task. Year One (2005) results were less than stellar. On key indicators, CCH ranked at the 12th percentile nationally. Rather than wallowing in self-pity, the CCH management team recognized this as a great opportunity. A systematic approach was developed to address employee opinion via departmental Action Plans. These Plans required employee participation and buy-in. Year One action plans totaled over 170. This may sound a bit staggering, but when you rank in the 12th percentile, there is a lot to improve.

Results of the 2006 survey showed dramatic improvements from a statistical standpoint, but still not where CCH wanted to be. In fact, CCH received HR Solutions national award for the most improved hospital by moving to the 39th percentile. Through the creation of additional measurable Action Plans, CCH ranked within the "Best Practice" range (90th percentile) in almost every single indicator in 2007. In many of the areas CCH was actually the best practice nationally! Maintaining that level of performance was the focus of this past years’ survey. In 2008 CCH achieved a national ranking of 97th percentile. So how did CCH achieve this performance? Well that's what you'll find out through this H2H Program. As you can imagine, there is no panacea, no magic bullet. The Action Plans were the most significant reason because employees were heavily involved in drafting and implementing the Plans.

The inaugural RWHC H2H (Hospital to Hospital) site visit for 2007-08 was held Friday, October 26, 2007 at Our Lady of Victory Hospital in Stanley with Cindy Eichman, President, as our host. The focus of this session is “Leadership Development.”

Cindy shared their experience related to leadership development. Sr. Lois Bush provided a brief history and rationale for Ministry Health Care's System approach to leadership development and coaching. Cindy shared how they do talent reviews and provide internal leadership development coaching to managers within the hospital. This includes identifying coaching needs for exemplary leaders and those in need of development. A panel of OLVH managers discussed their direct experience with the program. Participants left with resources to adapt this program locally, to share simple coaching tools for common challenges, and provide a few helpful website resources.

The Scalable Population Health Enterprise: Generating Clinical and Financial Returns from Cost-Effective Care Management

One of the central challenges for population health managers is extending care management past high-cost patients in a cost-effective manner. Hospitals and health systems transitioning toward population health management typically begin by focusing on the top 5% highest-cost patients.  With these high-cost patients comprising nearly 50% of total costs, this is a good place to start, but is insufficient for long-term financial success. Achieving sustainable margins under population health management requires extending care management beyond this cohort to the remaining 95% of the patient population. Prioritizing Population Health Interventions: From Data Aggregation to Actionable Insights

One of the most critical competencies of the population health enterprise is the ability to segment the population into actionable groups. Today’s data collection efforts provides an important first step to predicting future risk, but is largely insufficient for truly predicting future risk. Adding to the challenge, exhaustive data collection can become time-consuming and resource intensive. Despite the importance of risk analytics, only two-thirds of population health managers have the ability to stratify patients by risk. Successful population health managers use data and analytics to more effectively target their interventions. First, providers use existing clinical data to segment patients by individual disease states. Layering on claims-based analytics provides a more complete cross-continuum patient view. Systems fill in data gaps through partnerships and carefully targeted data collection, and then prioritize the right clinical interventions for each patient.

Ultimately, data for population health needs to answer 3 key questions:

1. Who are my riskiest patients?

2. Why are they risky?

3. What is the best intervention for them?
